Novel Non-Pharmacological and Lifestyle-Based Precision Approaches in Type 2 Diabetes Management: Integrating Technology, Diet, Exercise, and Behavioural Personalization

Main Article Content

Abstract

Background: Type 2 diabetes mellitus (T2DM) is a heterogeneous metabolic disorder in which lifestyle, behavioural, environmental, and biological factors interact to determine glycaemic control and long-term cardiometabolic risk. Although dietary modification, physical activity, weight management, and behavioural support remain fundamental components of diabetes care, conventional lifestyle prescriptions may produce variable responses between individuals. Precision lifestyle medicine seeks to address this heterogeneity by integrating individual metabolic characteristics, dietary responses, physical activity patterns, behavioural factors, and digitally generated health data into individualized interventions. Objective: This review synthesizes current evidence concerning personalized nutrition, structured exercise, continuous glucose monitoring (CGM), mobile health, digital therapeutics, telehealth, artificial intelligence (AI)-assisted behavioural support, and behavioural modification strategies for the management of T2DM. Methods: A structured literature review was undertaken in accordance with the principles of the PRISMA 2020 reporting framework. PubMed/MEDLINE, Scopus, Web of Science, and the Cochrane Library were considered for studies published between January 2015 and December 2025. Clinical trials, observational studies, systematic reviews, meta-analyses, and relevant evidence syntheses evaluating individualized or technology-assisted lifestyle interventions in adults with T2DM were considered. Studies involving type 1 diabetes, gestational diabetes, animal models, and interventions unrelated to lifestyle or behavioural modification were excluded. Evidence was synthesized narratively because substantial heterogeneity existed in intervention design, duration, personalization strategy, outcome definitions, and comparator conditions. Results: The evidence indicates that precision lifestyle interventions can improve several clinically relevant outcomes, although the magnitude and durability of benefit vary. Personalized nutritional strategies may improve postprandial glucose regulation, particularly when dietary recommendations are informed by individual glycaemic responses, food preferences, metabolic characteristics, and adherence patterns. Mediterranean-style and energy-restricted dietary patterns have demonstrated improvements in HbA1c, body weight, and cardiometabolic risk factors. Structured aerobic, resistance, and high-intensity interval exercise improve glycaemic control and physical fitness, while resistance training provides additional benefits for glucose disposal and metabolic health. CGM-guided behavioural interventions provide real-time feedback and may facilitate dietary and physical-activity modification. Digital applications, telemedicine, remote monitoring, and professionally supported behavioural platforms have generally demonstrated modest-to-moderate improvements in HbA1c, although intervention heterogeneity and variable engagement limit direct comparison. Behavioural approaches such as motivational interviewing can strengthen self-management and self-efficacy. Conclusion: Precision lifestyle medicine represents an evolving framework for individualized T2DM management rather than a replacement for established diabetes care. The most promising approach appears to be integration of personalized nutrition, structured physical activity, behavioural support, and appropriately selected digital technologies according to patient characteristics and preferences. Future research should prioritize longer follow-up, standardized personalization algorithms, clinically meaningful outcomes, health-equity considerations, and pragmatic trials evaluating sustained real-world effectiveness.
